ThrivePath prepares families, supports transition and follows outcomes at 4, 8 and 12 weeks.
Week 4 · Early adjustment
The first follow-up checks immediate safety, participation, relationships and whether the transition plan is being followed.
Week 8 · Stability
The second follow-up looks for patterns: whether routines are holding, concerns are being addressed and the placement remains appropriate.
Week 12 · Verification
The third follow-up determines whether safety and stability have been sustained long enough to record a verified outcome.
Responding to concerns
Follow-up is active protection. When risks emerge, RMFSL can strengthen support, revise the plan or work with authorised actors on a safer alternative.
Beyond the metric
The 12-week point creates a disciplined minimum for verification. It does not mean every family’s support needs end at week 12.
